By engaging in a telehealth consultation with Align Wellness Suite, LLC, you acknowledge and agree to the following terms:

Voluntary Participation: You are voluntarily participating in a telehealth consultation with Align Wellness Suite, LLC.

Nature of Telehealth Services: Telehealth involves the use of electronic communications to enable healthcare providers to deliver services to patients who are not physically present. This may include video conferencing, phone consultations, and electronic transmission of medical records.

Benefits: Potential benefits of telehealth include improved access to care, reduced healthcare costs, and the convenience of receiving care from your home.

Risks: Potential risks include technical issues such as interruptions or unauthorized access. Either you or your healthcare provider may discontinue the telehealth consultation if the connection is deemed inadequate.

Confidentiality: Your healthcare information may be shared with authorized individuals for scheduling and billing purposes. If others are present during the consultation, you will be informed, and you are expected to disclose any individuals present with you. All participants are required to maintain confidentiality. You have the right to:

-Omit sensitive details of your medical history or physical examination.

-Request non-medical personnel to leave the telemedicine examination room.

-Terminate the consultation at any time.

Alternative to Telehealth: You may choose to forgo telehealth services with Align Wellness Suite, LLC, and seek in-person evaluation elsewhere. Your participation in telehealth is entirely voluntary.

Limitations: Telehealth may have limitations regarding physical examinations. Certain assessments may be conducted by individuals at your location under the direction of the consulting healthcare provider or may not be performed at all.

Emergency Situations: Telehealth services offered by Align Wellness Suite, LLC, are not intended for emergency situations. In the event of an emergency or urgent medical issue, please call 911 or visit the nearest emergency department or urgent care facility.

Privacy Measures: To protect your privacy, do not share telemedicine login information or video conferencing links with unauthorized individuals.

By engaging in telehealth services with Align Wellness Suite, LLC, you certify that you have read and understand the above terms, including the risks and benefits of telehealth, and have had the opportunity to ask questions, which have been answered to your satisfaction.
